Laboratory Testing for Suspected Zika Virus for Suffolk County Residents
To: Health Care Providers (HCP):
What Is Necessary for Laboratory* Testing for Suspected Zika cases? Patient should take to the lab:
1. Lab Requisition or Prescription from HCP for Lab requesting:
Serum: Collect 6 mL whole blood, centrifuge, remove serum for a minimum of 3 mL
Urine: Collect a minimum of 3 mL of urine (no more than 20mL)
2. “NYSDOH Authorization for Zika Virus Testing” ticket generated by Suffolk County (SCDHS) Disease Control Clinical Staff - form will be issued via email to patient or fax to lab, or HCP if needed. Contact: Business Hours: 631-854-0333 and After Hours: 631-852-4820.
3. NYSDOH Infectious Disease Requisition (IDR) form**DOH-4463 (page 1 only) Completed by HCP: Patient Demographics, Submitter, Specimen Info, Clinical History. In Lab Exam Request section, specify Zika as agent, check “serology”, and add PCR to “Other”.
