Thank you for your interest in the Suffolk County Summer Youth Employment Program!

We understand that applications can be cumbersome and hope to aide you in filling out our application. Please note that there is specific information that you need to have on hand in order to be able to complete this application. Please read and follow the step-by-step instructions outlined below before you get started.


Step 1

Getting your Information Ready

We recommend having a parent / guardian on hand to complete this application as you will need to provide the following information:

  • Household size
  • Household income
  • Benefits/aide your family receives

If you currently do not live with a parent or guardian, you will have the opportunity to disclose your housing status to us for us to better place you in a worksite.

You will also need to provide certain documents that you will be requested to upload to our Document Center or physically bring into a Drop Off Location:

  • Copy of birth certificate
  • Copy of Social Security Card
  • If you are 18 or older Copy of Photo ID
  • If you are under the age of 18 you will need your original Work Card/Working Papers
  • 14 and 15 year old applicants need a blue card
  • 16 and 17 year old applicants need a green card
  • If you are not a citizen but a permanent resident, you will need to provide a copy of your permanent resident card, FRONT and BACK in place of your birth certificate
  • If you have a Work authorization card you will need to provide a copy of the front and back
  • If you are an 18 year old male or older you must register for selective service to participate in our program. If you have already received your letter of confirmation you must submit a copy. If you are uncertain of your status you can check here: Verify Registration | Selective Service System.

Step 2

Filling Out the Online Application

Once you have all this information and documentation together you are set to begin the application!

  • You will follow the application prompts filling in the information as asked. *Anything with an asterisk is required.
  • There are questions that are not mandatory. If the information asked about in these questions applies to you, please answer them.

After you complete the first page, you may provide your name & contact information.

You will be able to review what you have filled out before submitting.

  • Upon submission you will receive a Service Request Number.
  • Save this number! We recommend you write it down or take a screen shot immediately.

This is your application number and what you will use to upload the required documents and complete your application.

Step 3

Document Submission

Upon receiving your Service Request Number there will be a link at the bottom of this page that will take you to the Document Center where you will submit all the required forms. You will also receive a confirmation email to the email you provided in the application with a list and a copy of all the documents you need to fill out, print and sign. Here are the required documents to upload in the Document Center:

  • Birth Certificate
  • Social Security card
  • Photo ID (if you are 18 or older)
  • Permanent Resident Card (if you are not a citizen)
  • Work Authorization Card (if you are not a citizen or permanent resident)
  • Letter from DSS documenting your status in foster care

You will need to either fill in as a fillable PDF or print out and upload the following:

  • W-4
  • I-9
  • Payment Option
  • If you are under 16, parental consent form for payment option must be signed and submitted
  • Memorandum of Understanding
  • Retirement Form
  • Upload signed copy of parent/guardian consent page

Step 4

Document Drop-Off

The following documents will need to be dropped off either at the worksite you were referred by or at the designated drop of sites.

What to Drop Off

  • You will need to print out the parent/guardian consent page to be brought in.
  • If you are under 18 you will need to drop off your work card. We will return this to you at the end of the Program.

If for any reason you cannot upload your documents, you can drop off copies of everything and we will upload them.
