Freedom of Information Law (FOIL) establishes your right to access public agency records. If you wish to receive a copy of or view Suffolk County Department of Social Services (Department) records, submit an Application for Public Access to Records form here. To access a Spanish language form, click here. You may also fax the form to (631) 854-9996 or mail it to:
ATTN: FOIL OFFICER
SUFFOLK COUNTY DEPARTMENT OF SOCIAL SERVICES

Upon receipt of the form, the Department has five business days to acknowledge receipt of the request. If the Department approves your request, your records will be provided within twenty days. If the Department requires more than twenty days to provide the requested records, the Department will update you regarding delays and provide you with a date certain that the records can be provided.
The Department has the right to deny your request for access to records. Please review page two of the Application for Public Access to Records form for details.
If you request copies of an agency record, the statutory cost for reproduction is as follows:
- $.25 per page
- Actual cost of reproducing any other record.
NOTE: YOU MUST PAY THE APPLICABLE FEE, AND THE PAYMENT MUST BE PROCESSED BEFORE RECORDS WILL BE PROVIDED TO YOU.
If your request for records is denied you may submit an appeal in writing within thirty days of the date of denial. The FOIL Appeals Officer must respond to you in writing within ten business days of receipt of your appeal, explaining the reasons for further denial, or provide you with access to the record sought.
