Healthy Baby and Me
Healthy Moms equal Healthy Babies! The Perinatal Health Services Program provides pregnant, postpartum and parenting individuals with health education, linkage to insurance and medical care, connection to community resources and benefits programs, advocacy and support. Free and confidential services are provided in the client’s home by Community Health Workers who are knowledgeable, compassionate and empowering. Services are available to residents of Babylon, Brookhaven, Islip and Riverhead Townships. Bilingual workers available.
