COVID-19 Vaccination clinic – MODERNA - 6 months to 5 years & PFIZER ages 6 months to 4 years

CDC recommends COVID-19 vaccines for everyone ages 6 months and older.
First doses of the Moderna vaccine are available at this clinic to children ages 6 months through 5 years.
First doses of the Pfizer vaccine are available at this clinic to children ages 6 months through 4 years.
Appointments for this clinic are strongly encouraged. A limited number of walk-ins will also be accepted.

Vaccine supply is currently limited. Residents who have appointments will have priority.
All persons must show identification. A parent or legal guardian must provide consent for vaccination, either in person, by phone at the time of the vaccination appointment, or by completing a consent form. Link for consent form.
Note:
If your child receives the Moderna vaccine, they will need a second shot of the Moderna vaccine at least four weeks after their first shot.
If your child receives the Pfizer vaccine, they will need a second dose of the Pfizer vaccine at least 3 weeks after their first shot, and a third dose of the Pfizer vaccine at least 8 weeks after their second shot.
Medical practitioners will be on-site at all times at each clinic to answer any questions and tend to any needs or concerns.
