Departmental Mandatory Provider Compliance/ False Claims Policy

This policy applies to all Suffolk County Department of Health Services (SCDHS) employees, contractors, and agents. It outlines a compliance program, including the delineation of information regarding detecting and preventing fraud, waste and abuse (FWA), and informs staff regarding listed anti-fraud, false claim, and “whistle blower” protection status.

Immunization Record Request

The Suffolk County Department of Health Services does not maintain a vaccine registry for immunizations administered to Suffolk County residents by private physicians or other health care providers. The Department only maintains immunizations records for patients of the County Health Center system and for vaccines administered during our immunization clinics (e.g., Flu Vaccine to Senior Citizens, MMR to College Students, etc.)

In order to obtain copies of childhood immunizations, an attempt to locate missing records should be made by contacting previous health-care providers and searching for a personally held record. Serologic testing for immunity is an alternative to vaccination for certain antigens (e.g., measles, mumps, rubella, varicella, tetanus, diphtheria, hepatitis A, hepatitis B, and poliovirus). If records cannot be located, persons may or may not be susceptible and should consult with their physician as to the need for revaccination or blood tests demonstrating immunity.
