Apostille

NYS issues an Apostille (the legalization of a document for international use), the Suffolk County Clerk will certify copies for an Apostille.

If your document is a Birth, Death or Marriage Certificate and requires certification, send the document(s) to:

*Include a check or money order in the amount of $5.00 per document, made payable to the Suffolk County Clerk, also include a self-addressed stamped envelope.

For Authentication of a Notary Public's signature, send the document(s) to:

* Include a check or money order in the amount of $3.00 per document made payable to the Suffolk County Clerk, also include a self-addressed stamped envelope.
Note:
As a courtesy, if you provide a completed Apostille/Certificate of Authentication Request and a self-addressed, postage pre-paid envelope addressed to the appropriate New York State department together with payment to New York State, the Suffolk County Clerk’s Office will forward your document to New York State.
